To help prepare our Fraternal Survey for Supreme please complete the information requested below and return it at our next meeting or via email.
This information will assist us in determining the total number of hours of community service volunteered by our members.
Feel free to download and print the attached form, complete it (do not indicate your name) and drop it off in an envelope at the Church’s Community Center office to the attention of K of C mail folder or bring it to me at our next Council meeting.
